Mental Health Services in the Community - survey for service users, patients, carers and family members

The Community Mental Health Team in Angus (based at Whitehills HCCC in Forfar) has launched an online and anonymous survey regarding mental health services in Angus. 

This survey is for service users, patients, carers and family members. 

The Team wishes to know about the care and support services that you use in your local area. This includes community mental health teams, GPs, the NHS, and the voluntary and community sectors. They are looking for your views so they know what you need and can plan support that will make a difference to your lives. 

The survey should just take 5 minutes to complete and ends on Wednesday 18 April 2018.
